What not to do when designing a game

Well I was getting in a little bit of Star Trek Online this morning before I started to travel to my parents for the weekend. I was on a mission and I noticed a game mechanic that should NEVER be included in a game.

You should not have zero penalty for death then use that to make the game “Harder” by making it standard practice to die during missions and just keep coming back and whittling away at the enemies. Single player games figured out people don’t like that a long time ago. I really hope that it does not become standard practice in MMOs as death penalties go away.
